LWMB Travel Release Information
Travel Release Disclaimer
Whenever the LWMB travels together for an event, ALL LWMB students are expected to ride the school busses to and from the event.
In case of emergency, a parent or legal guardian can submit a “Travel Release” paper request form to your school’s Band Director.
No student will be allowed to leave with anyone but a parent or legal guardian from any LWMB event.
The LWMB Directors will approve or disapprove of all ‘Travel Release’ requests on a case-by-case basis.
This form is to be filled out by a parent or guardian only.

Instructions for Travel Release
- Read the itinerary for the event in question - CLICK HERE
- There are LWMB events where "Travel Release" WILL NOT BE ALLOWED. Please read the itineraries!
- Download the appropriate form above.
- A Parent or Guardian must complete the form.
- Bring the form to the event on the day of the event.
- You may turn it in via email, but this is only expected/required for overnight band trips. Please send forms to [email protected].
- After the conclusion of the event, meet at the assigned Travel Release location, at the assigned Travel Release time.
- LWMB students need to finish loading any uniforms or equipment and complete any other responsibilities before departing. LWMB students should not leave personal items on the busses, or transfer any personal responsibilities to another student.
- A parent or guardian must meet the assigned bus chaperone (based on the student's numbered bus assignment), present the "Travel Release" form, present a PHOTO ID, and then sign the chaperone bus roster to officially confirm the Travel Release.
- The LWMB Bus Chaperones are parent volunteers and are acting on instructions issued by the Band Directors. They may refuse to confirm your Travel Release if this protocol is not met, or if the Band Directors issue special instructions.
